What information is needed to sign up for an Oregon ABLE account?

To open an Oregon ABLE Savings Plan account, you'll need to provide some basic information as the account holder, including:

  • Full name

  • Contact information

  • Date of birth

  • Social Security number or tax ID

To finish setting up your account, you will be asked to:

  • Connect a bank account

  • Choose your investments

  • Fund your account (add at least $25)

If you’re an Authorized Legal Representative, you’ll need to provide the same basic information as the beneficiary. You may also need guardianship, conservatorship, or power of attorney documentation, depending on your relationship to the beneficiary.

If the beneficiary is not eligible for Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) benefits, they need a signed Diagnosis Form from a licensed physician. You will not be asked to provide this form during registration, but the IRS or the program administrator may ask for it at any time.
